E-Invoicing for Medical Clinics & Dental Practices in the UAE: Dates, Rules and Penalties
Most clinics and dental practices go live on 1 July 2027, with an Accredited Service Provider appointed by 31 March 2027. Practices at or above AED 50 million revenue go live on 1 January 2027. This page sets out what changes for consultations, procedures and insurance claims.

Below AED 50 million revenue: appoint an Accredited Service Provider by 31 March 2027, go live 1 July 2027. At or above AED 50 million: ASP by 30 October 2026, go live 1 January 2027. Government entities go live 1 October 2027. A voluntary pilot opens 1 July 2026.

What E-Invoicing Means for Healthcare Providers
Every medical bill becomes a structured XML document in the PINT AE format. It leaves your practice management system, passes through an Accredited Service Provider on the Peppol network, and reaches the buyer and the Federal Tax Authority from there. Nothing connects directly to FTA servers, and a PDF emailed to a patient is not an e-invoice. Your system must produce the format automatically, without exposing clinical detail.
What Changes for Medical Practices:
- Every Billable Service:
- Consultations, procedures, lab tests and dispensed items all need a structured e-invoice, not a printed receipt or a PDF
- EMR/Practice Management Integration:
- Your EMR or billing system connects to an Accredited Service Provider, which transmits on the Peppol network. It does not connect to the FTA directly
- Insurance Claim Handling:
- E-invoices must show full amount, insurance portion, and patient co-pay separately
- Medical Coding Integration:
- Whatever service codes you bill on, each one has to carry the right VAT treatment, because not every medical service is rated the same way

Healthcare-Specific E-Invoicing Challenges
1. Insurance Claim Complexity
Medical services often involve insurance. E-invoices must show total service cost, insurance coverage, and patient responsibility separately for accurate VAT and claim processing.
Example: Dental procedure AED 2,000. Insurance covers AED 1,600, patient pays AED 400. E-invoice must detail both portions with correct VAT treatment.
2. Medical Coding & VAT Categories
Not every medical service carries the same VAT treatment. Your system needs to apply the right treatment to each service code automatically, rather than leaving it to whoever raises the invoice. Confirm the treatment of your own service list with a tax adviser before you configure it.
3. Patient Confidentiality
E-invoices must comply with tax regulations while protecting patient privacy. Service descriptions should use medical codes, not sensitive diagnostic information.
4. Emergency Service Invoicing
Emergency treatment happens before anyone thinks about billing. Your system needs to queue those invoices and issue them once the patient is stable, rather than blocking care at the point of treatment.
5. Multi-Specialty Clinics
Patients seeing multiple specialists in one visit need consolidated e-invoices showing all services or separate invoices per department with proper linkage.

If You Cannot Issue Compliant Invoices:
AED 5,000 per month
The penalty is per month, not per invoice. It does not scale with patient volume, so a practice seeing 53 patients a day and one seeing five are charged the same. Left uncorrected for a full year it would reach AED 60,000, but the bigger exposure is the patients you cannot bill through a compliant system at all.
